Attend Team Care Challenge Luncheon

The PCPCC and the American Resident Project are proud to announce the Team Care Challenge Luncheon.This event will be the grand finale to PCPCC’s 2015 Fall Conference! During this luncheon, finalists will present their innovative ideas to an audience of conference participants and our renowned panel of judges.

The Team-Based Care Award

The Team-Based Care Award goes to the most innovative idea that promotes higher quality team-based care by involving the entire extended care team, including family and caregivers. Ideas will also offer solutions that improve patient outcomes and lower health care costs.

This award is sponsored by the Patient-Centered Primary Care Collaborative and The American Resident Project.

The Contest: Share Your Ideas

The purpose of this Challenge is to provide an opportunity for clinicians to be part of innovation. You can participate in this contest by submitting your team care ideas here. This website provides multiple resources including videos, images, text and an online forum for where you can submit your ideas and receive feedback from experts and peers.

Judges

Over 30 judges will be reviewing online submissions and selecting 5 finalists to present at the Luncheon. This panel of judges includes physicians and other clinicians, public health and policy experts, students, residents, and fellows.

Finalists will undergo an additional round of review at the live event by a smaller panel of judges and a winner will be selected.
