Biography

Tom Wroth, MD, MPH is Chief Medical Officer for Community Care of North Carolina (CCNC), a physician-led organization that builds and supports community-based healthcare delivery systems. CCNC currently manages North Carolina’s Medicaid population through medical home networks with robust care management and analytics tools. North Carolina is transitioning to Medicaid managed care in 2020 where CCNC will support independent primary care physicians through the Community Care Physician Network – a clinically integrated network with 850 practices and 3,000 clinicians.

Dr. Wroth practices as a family physician in a Federally Qualified Health Center (FQHC) where he cares for patients from cradle to grave. He is the former Chief Medical Officer of Piedmont Health, a system of ten FQHCs and two PACE sites. He was previously on faculty in the Department of Family Medicine at the University of North Carolina at Chapel Hill.  Dr. Wroth trained in Family Medicine and Preventive Medicine at UNC Chapel Hill.  He attended medical school at Columbia University, College of Physicians and Surgeons.
