Post-Conference Workshop on Advanced Primary Care: Emerging Models and Metrics to Achieve High-Value Care

November 5, 2019 from 1 pm to 3:30 pm, inclusive of lunch provided by the PCPCC.

Join with PCPCC Executive Members from across primary care to discuss emerging trends from commercial payers and measure developers. We will briefly hear from health plans and other innovators and through interactive sessions work together to define the next phase of advanced primary care. Do PCPCC members anticipate more alignment and reduced administrative burden? More “measures that (truly) matter”, and upfront resources for population health and care management? Help identify investment gaps and misalignments that PCPCC and our members can collectively address through our education, research and advocacy work.

Goals for the Workshop

  • Looking two to three years out, identify whether there is convergence on the direction of value-driven primary care payment models and measurement.
  • Define the top investments or changes practices must make transform from episodic care to population-based care delivery.
  • Define what support and advocacy can be provided by payers/purchasers, PCPCC, professional organizations to enable practices to succeed?
