Don’t miss this special address from the “godfather” of the patient-centered medical home and our Founding President Paul Grundy, MD, who will retire from the PCPCC Board of Directors at the end of this year. He is also the 2016 recipient of our most prestigious award, the Barbara Starfield Award, in honor of his life’s work and outstanding dedication to the medical home concept of care. Over the course of his career, he has worked tirelessly to promote and advance the PCMH model as a vehicle for transformational change in the way health care is delivered, not only in the United States, but around the world.
