AppleCare Medical Group’s Discharge Hospital Clinic

AppleCare Medical Group was formed in Lynwood, California, in 1996 by two practicing physicians, Dr. Vinod Jivrajka and Dr. Surendra Jain. Their vision was simple: to make quality care available locally to their community. Over the last 20 years, the multi-specialty medical group of independent practices has expanded into new communities, contracted with more hospitals and health plans, and added over 50 new doctors—but the mission has remained the same.

The Hospital Discharge Clinic is an exclusive clinic for members of AppleCare Medical Group who recently had a hospital stay. We encourage you to visit the clinic and will help you make an appointment before you leave the hospital.

Today, AppleCare Medical Group is comprised of hundreds of board-certified private-practice, independent physicians. Collectively, our physicians have been serving the Southeast Los Angeles County and Northern Orange County for decades, providing their medical expertise and passion to improving the health of our communities.
