Biography

Michael P. Jeremiah, MD serves as the Chair of the Carilion Clinic Department of Family and Community Medicine.  Carilion Clinic is composed of 8 hospitals and a group of more than 600 physicians serving the western and southwestern parts of Virginia.  The department of Family & Community Medicine is composed of more than 200 providers in 41 office sites and a family medicine residency program established in 1971. He is an Associate Professor of Family Medicine with the Virginia Tech-Carilion School of Medicine.  He completed his residency training at Carolinas Medical Center in Charlotte, NC before pursuing additional obstetrical training in Charlotte and a fellowship in faculty development through the University of North Carolina Department of Family Medicine in Chapel Hill, NC.  He served as Medical Director of the family medicine residency program for more than 10 years before being selected as chair.  His department and health system are aggressively pursuing strategies for more effective population health including the PCMH model of care (31 sites), partnership ventures with payors like Aetna, and a new ACO that is participating in a Medicare Shared Savings Program and other payment reform initiatives.
