Adrienne White-Faines, former CEO of the American Osteopathic Association and seasoned health strategist with experience in public health, community service, and physician practice management, will moderate this session on technology and innovation in primary care delivery models.  The panel will cover the use of activated data and predictive insights to align primary care services in a more personalized manner. And, we’ll showcase two patient centered approaches that address culture and gender specific health and social issues. We’ll also hear how a mobile app for collecting patient generated data is driving non-clinical interventions in a midwestern ACO’s SDOH/population health management program.
